Cognitive pathology made me laugh, as much of my work experience was in psychiatric practices.
Will add one additional possibility, some identify the "caring" they receive from healthcare as the only, or primary, positive human interaction they receive. Sadly.

Thanks, I keep pitching them; it’s nice to know when one connects.
There’s the old joke about the group of elderly women in the Dr’s office. One of them asks about the absence of one of their number. “She couldn’t come, she’s sick”.
The empty loneliness is heartbreaking. I see groups of young people sitting together, silently and sullenly looking at their phones, in search of the momentary empty calories of the next “like”, dreaming of being an “influencer” in the vacuous digital world. Fulfillment through service to each other is, for whatever reason, just outside their range of vision. Many will trudge on through their hollow, selfish lives and never really crack the spiritual code.

Wow. You're right about what brings fulfillment. And we get lost chasing "happiness".
